In order to receive your scholarship funds from our office for your MVS scholarship or Legacy Award, you'll need to download, complete and mail a copy of the Verification of Enrollment Form to our office.

If you're a 2018 MVS or Legacy scholar, you are not required to send in your high school transcript to receive your first payment.
